All parish ministers (volunteers, staff and clergy) are required to submit an annual background check (CORI) form with the Archdiocese of Boston. This includes liturgical ministers, faith formation, pastoral ministry, and parish leadership.

You are invited to download the current CORI form, print double-sided, and bring the completed form with you to save time. This is very helpful to us!

You may also come the Parish Office during business hours to submit your CORI forms.

New Archdiocesan Code of Conduct – Since February 2021

In addition to the annual CORI, the Archdiocese of Boston released a new Code of Conduct for all volunteer ministers in February 2021. You may download the new Code of Conduct and acknowledge receipt of it on this webpage. Staff members should contact Chris Kilburn for the Staff Code of Conduct.
